Not sure why you get that without looking closely on your site.
Possibly a conflict with other plugins.

Disabling sh404SEF fixed it.
Any chance to get those two working together?

Use SEF Advance.
If you want to send the admin password and FTP info I can take a look.

yep, SEF Advance would be an option if there was no encryption/ioncube loader needed. I'd tested it some years ago (it was on 1.x), it was pretty fast and very stable.
Because I'm not giving away access to my server/sites I have to set up a copy on another server as testing environment. Though I'm pretty busy atm, this will take some days. I'll take a look if there will be an update on your site, I don't think I'm the only one using this with sh404SEF, and will send you ftp and admin login as soon I'd set it up.
